Nurse at 104 W. Dorothy Lane, Kettering, OH 45429 US - Policy for Medication Administration at School

Policy for Medication Administration at School

MEDICATION POLICY - ST.  ALBERT THE GREAT SCHOOL

1.      Medication, whether prescription or over-the-counter, will be administered only if accompanied by the "Request for Administering Medication During the School Day" form, properly completed by the physician and parent/guardian/custodial parent.

2.      Medication must be in the original container and have an affixed label including the student's name, name of medication, dosage, route of administration and time of administration.

3.      New request forms must be submitted each school year, and as necessary, for changes in medication order.

4.      Medications and the signed permission forms must be turned into the school nurse or school personnel in the school office by the parent/guardian.

5.      Students may not keep any medication (prescription or over-the-counter) in their possession.  State law does allow a student to carry emergency medication, such as an inhaler or epi-pen, on themselves only if a physician has ordered such and this order is on file in the clinic.

6.      Students are permitted to keep cough drops in the clinic/classroom if accompanied by a note from the parent.
